Where is the Secret of Success in DBS Surgery?

Whenplacing the pacemaker electrodes in DBS surgeries, the most important point isto correctly locate an anatomical formation with a diameter of 2-3 mm in thebrain. Experts do not have deviations of even 1 mm when performing thisoperation. Pacemaker electrodes placed at the right point are among the mostimportant factors that directly affect the success of this surgery.

Here, the secret of success lies in the applicationcalled "Microelectrode Recording and Stimulation Technique." With theprinciple of awake, while talking surgery, we advance an electrode about 2microns thick into the brains of patients through a computer, and when weconnect it to a complex electronic mechanism, we can listen to the electricalactivity of a single cell in the brain.

Thus, we can monitor the  language and discharge characteristics of thecells in the region responsible for the disease we are looking for and tryingto reach, and we can locate these cells with an error margin of less than 80microns. At the same time, we can listen to the sounds of these cells, examinethe patient's responses to body movements during the surgery, and find theright place with precision

Measuring Patient Response to Electric Current

Thanks to the microelectrode recording and simulationtechnique, it is possible for us to evaluate the responses of awake patients toelectric current by applying an electric current at a microampere level, and atthe end of this process, we can draw the physiological map of the brain andfind the right place for the application.

Althoughhighly advanced imaging methods or computer calculations are used in thesesurgeries, the correct points for different patients also differ. The 1-2 mmdifference in the brain, which is different for each patient, means kilometersfor us. Here, this technique allows us to place the electrodes in the rightspots with less than an 80 micron margin of error.
